Advocacy & Specialized Support

Clear Guidance and Practical Coordination Through Complex Care

When a loved one is in the hospital, families often worry about more than updates and paperwork. They worry whether someone is making sure your loved one is hydrated, positioned properly, comfortable, safe, and not being left unattended for long stretches.

Heartfelt Health provides bedside advocacy and specialized support for families who need a trusted presence in the hospital. Our PSWs can be at the bedside helping monitor comfort, safety, positioning, hydration, and communication, while also keeping families informed and care organized.

Every caregiver and coordinator is carefully interviewed and screened. All interactions are documented, and communication is clear and transparent so families remain fully informed.

For families who require care aligned with halachic standards, we also provide culturally informed coordination so medical decisions and daily support remain consistent with religious values.

We proudly serve clients across Toronto and the Greater Toronto Area.

Two Unique Individuals Shaking Hands

Who This Service Is For

This service is designed for families who need practical support managing healthcare logistics, including:

Hospital Bedside Support
When a patient is in hospital and the family wants someone present to help monitor comfort, hydration, positioning, safety, and day-to-day wellbeing.

Hospital Stay and Discharge Support

When a hospitalization requires both bedside support during the stay and a clear, organized plan for returning home safely.

Ongoing Medical Appointments and Referrals
When multiple providers, specialists, or assessments must be scheduled and tracked.

Halachic-Sensitive Care Planning

When care arrangements must align with halachic guidance and religious observance.

Complex or Changing Care Situations
When evolving medical needs require oversight and coordination to avoid gaps.

Support can be short-term during a transition or ongoing when care remains complex.

How We Support You

When a Loved One Needs Someone at the Bedside

Hospital staff work hard, but they cannot provide one-to-one attention at all times. Our PSWs can be present at the bedside to help make sure your loved one is hydrated, positioned properly, comfortable, calm, and not left struggling alone.

When Safety and Comfort Need Closer Attention

A hospital stay can be physically and emotionally exhausting, especially for older adults or vulnerable patients. We help reduce risk by paying attention to positioning, time spent sitting or lying in one place, safe movement, basic comfort, and signs that the patient may need support right away.

When Families Cannot Be There Every Hour

Many families are balancing work, children, distance, or religious obligations and cannot stay in the hospital around the clock. We provide a trusted in-person presence so someone is there to notice what is happening in real time and keep the family informed.

When Communication Needs to Be Backed by Real-Time Observation

Because our support is provided at the bedside, we are not just repeating updates second-hand. We help families understand what is happening based on direct observation, organized notes, and clear communication about comfort, routines, and concerns that may need follow-up.

When a Hospital Stay Turns Into a Transition Home

Support does not stop when discharge begins. We help families understand instructions, prepare for the return home, and coordinate PSW care and follow-up support so the transition is safer and less overwhelming.

When Religious Observance Requires Careful Attention

Halachic and culturally sensitive coordination can be integrated into any form of support we provide, whether in hospital, facility, at home, or synagogue so your practical plan and religious standards stay aligned. We support communication between families, healthcare providers, caregivers, and rabbinic guidance where appropriate so care remains respectful and aligned with religious values.

What This Service Does Not Include

In hospital and community settings, our role is to provide non‑medical bedside support, practical oversight, and communication assistance alongside the clinical team. We do not replace or duplicate the responsibilities of physicians, nurses, or other regulated health professionals.

This service does not include:

  • Clinical assessment, diagnosis, or treatment decisions.

  • Prescribing, administering, or adjusting medications.

  • Making or overriding medical decisions on behalf of the patient or family.

  • Legal, financial, or insurance advice.

  • Acting as a formal substitute decision‑maker or power of attorney.

Our focus is on comfort, safety, organization, and advocacy within the care plan that you and your healthcare providers have chosen.

What Bedside Support Can Include

Depending on the situation and hospital setting, support may include:

  • Helping the patient sit properly in bed or in a chair for comfort and safety.

  • Encouraging hydration and helping the patient with meals or drinks when appropriate.

  • Assisting with repositioning and comfort measures to reduce prolonged pressure and discomfort.

  • Providing calm companionship and supervision so the patient is not left alone in distress.

  • Noticing changes in comfort, alertness, confusion, or mobility and alerting the family or hospital staff.

  • Supporting toileting, hygiene, and other non-medical personal care needs where appropriate.

  • Keeping families updated when they cannot be physically present.

  • All bedside support is non‑medical and provided alongside, not instead of, the hospital’s nursing and medical care teams.

  • Helping prepare for discharge and the return home.
