Adults with Disabilities Support
Overview:
Our PSWs support adults with physical, developmental, or cognitive disabilities to live independently with dignity and safety at home.

Each individual we serve has unique needs, preferences, and goals. Our PSWs build personalized care plans that adapt to those realities while helping clients maintain as much autonomy as possible. That includes:
1. Personal Care and Daily Living Support

PSWs assist clients with everyday activities that may be difficult or unsafe to perform alone. This includes:
-
Bathing, showering, grooming, and dressing
-
Toileting and incontinence care
-
Oral hygiene and skin care
-
Assistance with mobility, including transfers and use of assistive devices
-
Encouraging and supporting client-led participation where possible
We work with respect for each person’s comfort and routine, taking the time to understand how they want to be supported.
2. Meal Preparation and Feeding Assistance
Nutrition is essential, but preparing meals or eating independently may present challenges. Our PSWs help by:
-
Preparing meals according to dietary needs or restrictions
-
Supporting adaptive feeding techniques or utensils
-
Offering reminders and encouragement during meals
-
Monitoring intake and noting any feeding difficulties
-
Ensuring safe food storage and clean-up
This support helps promote health and a sense of normalcy at mealtimes.
3. Mobility and Physical Assistance
For individuals with physical disabilities, our PSWs help maintain safety and comfort by:
-
Assisting with transfers from bed to wheelchair or toilet
-
Supporting the use of mobility aids like walkers or braces
-
Preventing falls and responding to mobility-related fatigue
-
Helping with light exercise routines when recommended by therapists
-
Encouraging movement to prevent stiffness or circulation issues
We focus on safety and empowerment, always moving at the client’s pace.
4. Companionship and Emotional Support
Living with a disability can be socially isolating, especially when individuals live alone or have limited social networks. Our PSWs provide:
-
Friendly, consistent companionship
-
Participation in hobbies, activities, or light outings
-
Gentle conversation and emotional support
-
Encouragement to engage in community, creative, or spiritual activities
-
Building rapport through respectful, trust-based relationships
Human connection is a key part of mental and emotional health, and we prioritize it in every care plan.

5. Household and Routine Support
Keeping a household running smoothly often requires support. PSWs assist with:
-
Light housekeeping (tidying, laundry, kitchen cleanup)
-
Grocery assistance, meal planning, or shopping (where applicable)
-
Garbage disposal and basic home upkeep
-
Organizing personal items to improve independence
-
Helping with scheduling or reminders for appointments and medications

This support helps clients feel more in control of their environment and daily life.
6. Supporting Family and Care Teams
Many adults with disabilities have care teams or involved family members. Our PSWs:
-
Collaborate with family, social workers, or healthcare providers
-
Follow structured care plans and communicate updates
-
Observe and report changes in behavior, health, or routine
-
Respect client autonomy while providing transparency to families
-
Adjust care as needs or goals evolve
Our role is not just to provide help, but to be a trusted part of the wider care network.
